We believe every person deserves a stable, supportive home where they can thrive. By partnering with counties, case managers, and providers, we help ensure that individuals with the most complex needs receive the thoughtful, well-prepared placements they deserve.

The Placement Assistance Program at Hope Rises of MN is designed to support counties and providers in securing successful, sustainable placements for individuals who have not thrived in traditional Community Residential Settings (CRS). Many individuals with high behavioral acuity, complex trauma histories, or intensive support needs find themselves cycling through hospitals, crisis homes, or temporary stabilization programs simply because an appropriate long-term placement is unavailable. Our program was created to change that.

 

A Specialized Approach for Complex Cases

With more than 30 years of combined experience, our leadership team, Adam Fox and Ashley Voelker, brings deep expertise in disability services, organizational development, and 245D program design. Over the past decade, our work has focused almost exclusively on developing CRS settings for individuals with complex behavioral profiles, high supervision needs, and long histories of placement disruption.

This experience has shaped a placement model that prioritizes stability, readiness, and long-term success.

How the Program Supports Counties and Providers

The Placement Assistance Program offers hands-on support throughout the entire placement process, including:

  • Identifying providers equipped to support individuals with high behavioral or medical needs.

  • Assisting with matching, transition planning, and provider readiness assessments.

  • Helping teams understand the individual’s history, strengths, and support strategies.

  • Providing consultation to ensure receiving providers have the tools, training, and structure needed for success.

  • Reducing placement breakdowns by strengthening the foundation before the move occurs.
