Initial Organizational Assessment
Initial Organizational Assessment (1 full day or more, depending on organizational size/structure) – This assessment includes an overall review of the organization. It includes everything from reviewing policies and procedures, 245D compliance assessment, financial processes and procedures, employeeand HR policies, organizational online presence/marketing recommendations, etc. After consultation, review, and assessment, a full report of recommendations will be provided to you. This will allow you to prioritize how Hope Rises can be most impactful for your organization.
245D Compliance Support & Auditing (Hourly or Project-Based)
Individual audit – includes individual full programming documentation/books.
Program audit - includes an individual’s documentation/books at the site. CRS receipt of the site, PAPP (Program Abuse Prevention Plan), Financial Audit, and Staff Meeting and Training.
HR and Employee Files Audits (Hourly). An audit and recommendations on the overall standards for employees’ files.
Application Assistance/ Training Completion (Hourly). This includes preparation and consultation of all documentation required to file for your 245D license; Preparation and consultation of all documentation required to enroll with HCBS; a personalized handbook of up-to-date policies and procedures required to establish your 245D business; and submission of your licensing and enrollment documentation to the respective governing entities.
Training
Training on Annual/Intake meeting preparation (Hourly or Project-Based). This training is to teach the provider and their management team how to meet the 245D requirements for completing annual and intake paperwork at intake meetings.
Training for New Designated Coordinators & Designated Managers (Hourly or Project-Based). This training includes types of services, rights and restrictions, health needs, protection standards, emergency use of manual restraints, assessment development and implementation, progress reports and meetings, staff training, and DC and DM qualifications and responsibilities. Through discussion, we will explore what each area means for service providers and how participants can ensure compliance with 245D standards within their companies. This training can be done in a one-on-one or group setting.
Positive Support Rule 9544 Training (8 hours) – This training meets the requirements assigned in the Positive Supports Rule 9544 and 245D initial training requirements for direct support employees. This training is available with or without Physical Intervention Training.
Crisis Intervention Training (8 hours) – This training is not part of the required 245D training, but an additional training to help employees work with individuals who display significantly challenging behaviors. Topics covered include team building, proactive strategies for supports, reactive strategies for supports, supporting people in crisis, trauma-informed care, Maslow’s hierarchy of needs, and a refresher on physical interventions (should have Positive Behavioral Support Training prior to this class).
Physical Intervention Training (2 hours) – This training involves learning to manage crisis situations safely and effectively through safe physical intervention.
On shift training with staff (Hourly) – On shift training and observation for employees to help build relationships built on trust, work effectively with co-workers, provide appropriate positive support, identify patterns/problems,s and help resolve them.
Behavior Support and Programming
Behavioral Observation and Behavior Support Plan writing (Hourly) – On-site observation/assessment of individuals receiving services that are displaying behaviors above and beyond what the provider can handle. Consider this option before issuing service suspension or termination notices to individuals exhibiting difficult-to-manage behavior.
Positive Support Transition Plans (Hourly) – Completion of Positive Support Transition Plans for individuals who reach the threshold requirements for these plans. This includes the initial writing of the plan and training of staff on the initial plan. Any revisions, quarterly reporting, etc., will be the provider’s responsibility or can be requested at an additional cost.
Program/Outcome review, revision, and development (Hourly) – We are willing to review existing programming for people receiving services and make recommendations for changes/improvements. If desired, we can develop person-centered Outcomes and Data Tracking for people receiving services that are compliant with requirements under 245D.
Organizational
Organizational System assistance (Hourly or Project Based) – Identifying process, policies, and procedural changes to adhere to licensing standards, as well as improve systems and efficiency of the organization.
Quality Assurance (Hourly or Project-Based) – recommendations on overall standards for employees, management, and senior leadership of the organization. Coaching on how to provide different services in accordance with MN DHS licensing and service standards, with a person-centered and quality-focused approach.
Financial Review & Assistance (Hourly or Project-Based) – This may include writing DWRS exceptions, building budgets, addressing organizational P&L issues, assisting with collecting earned revenue, navigating MHCP provider issues, etc.
Special Project Management and Implementation (Hourly or Project-Based). Advises organizations on project implementation from start to finish; performs tasks such as defining project scope, setting goals and milestones, developing schedules, managing resources, and ensuring on-time, within-budget delivery. This includes the HR/ Payroll system, CRM system, and online documentation system.
